KINDERGARTEN AND NEW STUDENT REGISTRATION
When enrolling your child/children, please bring the following documents to the school office:
- Birth Certificate;
- Immunization Records;
- Proof of Residency (Deed or Lease in name of the custodial parent/guardian AND current Utility Bill (gas
& electric, phone, water) in the name of parent/guardian for service at the NCH residence).
*For families living with relatives or friends, both the deed/lease and a current utility bill are required for the legal NCH resident, and a notarized affidavit filled out and signed by both legal resident and parent/guardian of child requesting enrollment, AND current official mail addressed to parent/guardian at the NCH address.

BEFORE AND AFTER SCHOOL CHILD CARE
